Additional Forms

These additional forms are to be completed only if it applies to your business:

• Applicants with multiple Shareholders, Officers, Directors, Partners, please download additional copies as needed of the:
  • Key Stakeholder Declaration Form and Particulars of Key Stakeholders Form
• If a shareholder is not an individual, e.g., corporate entity, please complete the Corporate Shareholder Information Form.
• If you will be operating from a residential location, please complete the Terms and Conditions - Operating from Residential Location.

Background Check / Criminal Record Check (CRC)

Your CRC can be obtained from some OPP detachments or most Municipal Police Services. A CRC can also be obtained from an online background check service provider. TICO has partnered with Sterling Backcheck / MyBackCheck.com to assist applicants with obtaining this report.
